Informed Consent Forms and Weekly Safety Testing

Dear LS Students, Families and Staff,

Thank you for your continued support as we kick off the 2021-2022 school year. At Lincoln Sudbury keeping staff and students safe and in school is our top priority.

Arranged through the MA Department of Elementary and Secondary Education we are offering weekly routine COVID safety checks (previously referred to as pool testing) to prevent the spread of the virus undetected among students who are not showing symptoms. We will be collecting nasal swab samples in school every Tuesday starting September 21. Each participant will submit TWO swabs. The first swab is used for the pooled test and the second swab is only used by the lab if a pool tests positive in order to determine which individual in the pool is actually positive. The tests are not uncomfortable and easy for students to do themselves.

Any student or staff person can participate but only if a permission form has been filed on their behalf. To participate in the program,this form Parent/Guardian Link must be submitted on behalf of any student who is younger than age 18. For students 18 or older and staff this form Staff/Adult link must be used and signed by that individual. Please submit the required form by noon on the Monday prior to the Tuesday sample collection date. There is no charge for participating. We expect to receive results within 48 hours of sample collection.

Any individual who tests positive will be informed directly. If a student tests positive for COVID-19, they must isolate at home for the CDC-recommended ten days since symptoms appeared or ten days since the positive test before returning to school. The student must also be without a fever for 24 hours.
