Transcript Request Information
In order to personally receive or send a copy of your high school transcript, you will need to complete the transcript request form. You can mail or fax (978-443-0118) your request. You can also email your request to registrar@lsrhs.net Please be sure to include all of the information requested in the form.
There is a $5.00 charge for each transcript you order. Scholarship and military academy transcripts are always free.
Current Lincoln-Sudbury Regional High School students should see their guidance counselors or stop by the Registrar's office for transcript information.
